Osteoporose, vitamine D geen extra invloed

Botontkalking, ook wel osteoporose genoemd, houdt in dat botten brozer worden. Deze aandoening komt vaker voor bij vrouwen dan bij mannen en begint meestal op latere leeftijd. Zo heeft 1 op de 3 vrouwen boven de 60 jaar last van osteoporose.
De overgang bij vrouwen vergroot de kans op het krijgen van deze aandoening. Na de overgang maakt het lichaam namelijk minder vrouwelijke hormonen aan. Terwijl deze vrouwelijke hormonen oestrogeen en progesteron juist er voor zorgen dat de afbraak van botten geremd wordt. In de jaren na de overgang breekt het lichaam de botten dus sneller af, waardoor ze brozer worden. De grootste kans op osteoporose is wanneer vrouwen voor hun 45e in de overgang zijn gekomen.
Vitamine D geen extra invloed
Er wordt vaak geadviseerd om voldoende calciumrijke producten en vitamine D in te nemen. Uit onderzoek van het Winthrop University Hospital [1] blijkt dat Calcium inderdaad de botgezondheid bij vrouwen na de overgang verbeterd, maar voor vitamine D geldt dit niet. Onderzoeker John Aloia legt uit dat Vitamine D en calcium het botmetabolise (proces van botaanmaak en botafbraak) beïnvloeden door de hoeveelheid hormoon uit de bijschildklieren (PTH) te verminderen. Hierbij is vitamine-D enkel gunstig bij vrouwen die een vitamine-D te kort hebben. Bij vrouwen die de dagelijkse hoeveelheid voedingsstoffen en vitaminen binnen krijgen, geeft dit supplement geen enkel voordeel, meldt Aloia.
John Aloia bestudeerde met zijn collega’s 159 vrouwen na de overgang. De groep vrouwen werd onderverdeeld in vier groepen. Zo kreeg de eerste groep vitamine D en calcium toegewezen. De andere groep kreeg alleen vitamine D, een groep kreeg alleen calcium en de laatste groep kreeg placebo’s. Over een periode van zes maanden werd de botgezondheid van de vrouwen onderzocht. Hieruit is gebleken dat de natuurlijke botafbraak bij de groep vrouwen die calcium slikten, aanzienlijk minder is, terwijl vitamine D dit effect niet had. Aloia meldt ‘’Ons onderzoek suggereert dat vitamine D boven de aanbevolen dagelijkse hoeveelheid geen extra invloed heeft op de botgezondheid, voor calcium geldt dit wel’’ [1].
Gebruik de juiste voedingsmiddelen?
Naast het voldoende innemen van Calcium is het belangrijk om te weten dat bepaalde voeding de opname van calcium in de weg kan zitten. Voedingsstoffen als suiker, alcohol en cafeïne zorgen ervoor dat er minder calcium wordt opgenomen in het lichaam.
Botmetabolisme (proces van botaanmaak en botafbraak) vervangt jaarlijks circa 15% van de botmassa bij gezonde volwassenen. In een studie [2] is bevestigd dat onevenwichtige verhoudingen van botafbraak en botopbouw leiden tot meer botverlies dan botformatie. De studie bevestigt dat inname van uitgekiende aminozuren, de osteoblasten (botvormende cellen) activeert en botafbraak vermindert.
Een in vivo studie, waarin osteopenie (lage botdichtheid) bij postmenopauzale vrouwen is nagebootst met dieren, bevestigt een overeenkomstige botmineraaldichtheid (BMD) in de ovariëctomie groep die gedurende 12 weken is gevoed met aminozuren (eiwitten) ten opzichte van de non-ovariëctomie controlegroep die niet is gevoed met deze aminozuren [3].
In dezelfde studie [3] is de dagelijkse inname van genoemde aminozuurverbinding onderzocht ter zake het effect op botgrootte, botsterkte en botstevigheid. De resultaten bevestigden in de ovariëctomie groep die gedurende 12 weken is gevoed met deze aminozuurverbinding een verhoging van deze parameters ten opzichte van de non-ovariëctomie controlegroep die niet is gevoed met deze aminozuren.
Voorts wordt In talrijke studies [4] bevestigd dat door dagelijkse inname van uitgekiende aminozuren, de botcellen die belast zijn met botvorming (osteoblasten) voorkeursmatig worden gestimuleerd, vergeleken met cellen die betrokken zijn bij botafbraak (osteoclasten), hetgeen leidt tot een toename van botvorming.

Bewegen
Naast juiste voeding en vitaminen kan voldoende bewegen ook helpen om osteoporose te voorkomen. Zo zorgt bewegen (of lsporten) er voor dat de botten en spieren sterker worden.
Het verlies van botmassa kan vertraagd worden door lichaamsbeweging. Een half uur per dag bewegen is daarom belangrijk. Het gaat beslist niet om sporten op topniveau. Elke vorm van lichaamsbeweging is zinvol. Beweging waarbij het lichaam met zijn eigen gewicht wordt belast, heeft het grootste effect. Denk hierbij aan wandelen, traplopen, tuinieren, springen, hardlopen.
Gewichtsondersteunende lichaamsbeweging, zoals zwemmen en fietsen, heeft minder effect. Vooral zwemmen is minder effectief, omdat het lichaam dan niet echt wordt belast; het wordt gedragen door water. Wel maakt zwemmen de spieren en gewrichten los, zeker zwemmen in warm water.
